This team is skilled in planning, matching people to service and to identify the supports necessary to achieve their personal goals.<\/p>\n<p style=\"text-align: justify\">The Planning and Coordination team supports individual and family choice by working together to coordinate efforts to highlight options, research potential resources, and promote community involvement.<\/p>\n<h3>Services for Children<\/h3>\n<p>An application must be submitted for\u00a0eligibility determination.
