Community Living North Bay offers two transition coordinators that are here to help facilitate the \u2018transition\u2019 from high school to adulthood.\u00a0 A Transition Coordinator can help youth to plan for their future and then support them in connecting them with the appropriate community members, agencies, programs and funding.\u00a0 Topics that are commonly identified as areas needing support are: education, vocational opportunities, community connections, independent living, rights and advocacy.<\/p>\n<p style=\"text-align: justify;\">The role of the Transition Coordinator is to collaborate with the youth, their families, school representatives, and other support personnel to assist them through the transition planning process. They will draw upon strengths and contributions of each team member to ensure a coordinated effort.\u00a0 The coordinator will provide leadership to guide the team and youth\/family to accomplish the outcomes set out by the plan.<\/p>\n<h2>Who Qualifies for Community Transition Services?<\/h2>\n<p>Youth with an intellectual disability.
