Community Living Individualized Funding Services<\/u><\/strong><\/p>\n

Community Living North Bay \u2013 Individualized Funding, is a perfect blend of business, and individualized supports. We want to provide the best customer service possible, while keeping in mind the need of each individual.<\/p>\n

We can assist managing different types of funding, including, but not limited too:<\/p>\n

Passport funding<\/u> is available to adults 18 years or older who have an intellectual disability and their families. Community Living assists individuals and families to manage their Passport funds in the way that person wants, within the Passport Funding Guidelines, and budget outlined for the person.<\/p>\n

Specialized Accommodation Funding<\/u> \u2013 Funding applied through Community Living North Bay Clinical department, Individualized Funding would co-manage the funds, provide staff if needed, to meet a clinical goal, (one example, is transitioning to a Residential setting).<\/p>\n

Urgent Response<\/u> \u2013 When a situation is deemed a crisis, at the DSO level, urgent response funding can be accessed to assist individuals\/families.<\/p>\n

 <\/p>\n

Passport funding can be used for the following services and supports:<\/strong><\/p>\n

Community participation \u2013 YMCA, day programs, going to movies,<\/p>\n

Activities of daily living- learning to cook, learning literacy skills, Driver license assistance, etc.<\/p>\n

Caregiver respite- Caregivers in need of relief<\/p>\n

Person directed planning \u2013 if you don\u2019t know what you want to use your funding on, planning can help!<\/p>\n

\u00a0<\/strong><\/p>\n

Goals of Passport<\/strong><\/p>\n

Foster independence<\/p>\n

Increase participation in community<\/p>\n

Promote social inclusion<\/p>\n

Support families and caregivers<\/p>\n

Help young people transition from school to university<\/p>\n

Community Living North Bay- Individualized funding has assisted all these situations, and more.<\/p>\n

 <\/p>\n

When services are purchased from Community Living North Bay- Individualized Funding, these services are based on the three models listed below:<\/p>\n

Staff Model<\/strong><\/p>\n

The staff model is a full agency managed approach to service delivery. Community Living North Bay will assist by:<\/p>\n

Providing employees who are fully trained, have access to all agency resources and are experienced with supporting people with complex and challenging needs.<\/p>\n

Hours of work and performance would be supervised by a Community Living North Bay Manager. Assisting with all invoicing, budgeting and administrative requirements.<\/p>\n

Purchase for Services<\/strong><\/p>\n

Individuals who don\u2019t require staff support, but are looking for assistance with purchases, memberships, planning, or the reimbursement side of Passport. We will assist the individuals managing funds the way they want.<\/p>\n

Person Directed Planning<\/strong><\/p>\n

Person directed planning and facilitation is a process which can be purchased for people with intellectual disabilities. Planning will vary depending on the needs of the person. Person directed planning and facilitation is about supporting independence, inclusion and choice.<\/p>\n
